New Patient Forms

     Please take a moment prior to your scheduled appointment to complete your new patient forms and registration online. Please make sure to complete both the “Patient Information & Health History” and “Consent & Acknowledgements”. This information is confidential and will be securely stored directly into your personal chart.

      Although it’s not required, completing the forms beforehand will save you time so we may better assist you in a timely manner.  If you have any problems with the forms, be sure to call our office and we will gladly send them by mail, or fax them to you.

Your First Appointment

     At Oak Tree Family Dentistry, your initial appointment will consist of a consultation explaining your diagnosis and treatment options.  All areas of your mouth will be thoroughly examined. This will include a detailed examination of the teeth, oral cancer screening, supporting structure alignment, bite, and more. A complete set of x-rays will be taken and models of your teeth will be made if necessary. All the information that we obtain will be carefully evaluated so we may accurately assess your dental needs.

     Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays if applicable
  • A list of medications you are presently taking
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during the consultation visit.

Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.); a list is helpful.

Preventive Hygiene Appointment

After your examination, you will be ready for the preventive oral hygiene session. At this time, you will have access to the latest theories and techniques for controlling dental disease. Specific approaches will be recommended for your individual needs. Your teeth will also be cleaned by our skilled dental hygienist. At Oak Tree Family Dentistry, we consider this session to be one of the most valuable services we offer to our patients.

Comprehensive Dental Examination

After your preventive appointment, we will explain your current oral health and plan for treatment, including immediate needs and long-term goals. Our recommendations will be tailored to your specific needs and objectives.

Treatment Plan

Finally, our treatment plan coordinator will review, in details, your custom set of goals to improving your oral health; and more importantly, your overall health. As a courtesy to our patients, we will prepare all of the necessary insurance forms. However, we remind you that your policy is an agreement between you or your employer and your insurance company, not between your insurance company and our office.

We can make no guarantee of any estimated coverage, but we will do our best to see that you receive your maximum benefits. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. If you would like to know what your expected coverage will be, we will submit a pretreatment estimate. Your insurer will generally send a detailed response within four to six weeks.

When extensive dental care is necessary or when dental insurance is involved, financial arrangements can be made with our office team. Please inquire about financing plans. Our primary concern is your dentistry, and we will be sensitive to your financial circumstances.
